Wage Range: $45 - $55 DOE

Setting & Populion Served

Life Care Centers of America facilities opere as Skilled Nursing Facilities (SNFs) th provide:

• Long term care for residents who require ongoing skilled support and compassione daily assistance
• Short term, sub-acute rehabilition for pients recovering from surgery, illness, or injury
• Collaborive care through an interdisciplinary team approach with nursing, therapy, and medical staff working together
• A diverse pient populion including individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs

Pient mix varies slightly by facility. As a therapy professional, you will help pients achieve functional progress and improve quality of life within a supportive, pient-centered environment.

Position Summary

The OT - Occupional Therapist provides direct rehab care in occupional therapy to pients upon physician referral in accordance with all applicable laws, regulions, and Life Care standards.

Educion, Experience, and Licensure Requirements

• NBCOT certified upon hire, but renewal is optional going forward
• Gradue of an accredited program in occupional therapy (BSOT or MSOT)
• Currently registered/licensed in applicable Ste. Must maintain an active license in good standing throughout employment.
• One (1) year experience preferred; new grads welcome to apply
• CPR certificion upon hire or obtain during oriention. CPR certificion must remain current during employment.

Specific Job Requirements

• Participes in community health mters/projects as approprie
• Proficient in Microsoft Word, Excel, and e mail
• Liaisons with pients, families, support departments, etc., to adequely plan for pient needs
• Must demonstre good body mechanics all times
• Make independent decisions when circumstances warrant such action
• Knowledgeable of practices and procedures as well as the laws, regulions, and guidelines governing functions in the post acute care facility
• Implement and interpret the programs, goals, objectives, policies, and procedures of the department
• Perform proficiently in all competency areas including but not limited to: pient rights, and safety and sanition
• Maintains professional working relionships with all associes, vendors, etc.
• Maintains confidentiality of all proprietary and/or confidential informion
• Understand and follow company policies including harassment and compliance procedures
• Displays integrity and professionalism by adhering to Life Care’s Code of Conduct and completes mandory Code of Conduct and other approprie compliance training

Essential Functions

• Follow occupional trement plans for pients (i.e., activities of daily living)
• Establish, assess, and modify realistic, measurable, timely, and functional goals
• Oversee and evalue care given by OTAs, Rehab Aides, and students
• Chart appropriely and timely
• Utilize therapy software appropriely and accurely
• Exhibit excellent customer service and a positive titude towards pients
• Assist in the evacuion of pients
• Demonstre dependable, regular tendance
• Concentre and use reasoning skills and good judgment
• Communice and function productively on an interdisciplinary team
• Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
• Read, write, speak, and understand the English language
