Serenity Supportive Care is seeking a Palliive Care NP for our Eugene, Oregon team.

Schedule: Monday-Friday, 8am-5pm (No on call. About 20 visits per week.)

Wh You’ll Do:

• Assume primary responsibility for determining pient eligibility for care based on Ste, Federal guidelines.
• Obtain da on physical, psychological, social, and spiritual factors th may impact pient and family’s needs and initie intervention by other members of the interdisciplinary team
• Initie communicion with the tending physician, other interdisciplinary staff members and other agencies for the purpose of admission or recertificion for care and coordining optimal care.
• Effectively utilize resources to assist in meeting the pient’s needs
• Provide regular visits to reassess the pient’s eligibility, needs, and upde the plan of care

Qualificions:

• Gradue from an accredited registered nursing program
• Licensed as a Nurse Practitioner in the ste of practice
• Minimum of two years of varied work experience as a professional nurse
• Must possess current CPR Certificion
• Demonstred skill in nursing practices and clinical judgments
• Ability to make home visits and provide nursing care in the home environment
• Ability to cope with family emotional stress and tolerance of individual lifestyles
• Interpersonal, verbal, and written communicion skills necessary to complete job responsibilities
• Valid driver’s license and proof of insurance is required
