Ernest Henry Elsner was my grandfather. According to this form, he participated in the "World War" (not referred to as World War I because the second one hadn't happened yet) as a private with "Troop K" of the "3rd U.S. Vol. Cavalry."
• He was born in 1878, so was 20 at the time of his enlistment, May 14, 1898, in Lincoln, Nebraska.
• The eventual "Nature of Discharge" is shown to be "services being no longer required."
• The application is dated 1928, so I guess he didn't pursue this until he was 50 years old.
• The official stamp is hard to read but indicates "Superior Court of Santa Ana, California."
• The text on the back explains the importance of filling it out immediately to gain rights to hospitalization of the signee as well as participants in the "Spanish War."
The form is in really good condition for the 90+ years with the expected age-yellowing and paper fatigue at the folds.
